Orientations
The following orientations will be held at our Mt. Zion, Illinois clinic (440 Sunset Ct., Suite B) at 6 p.m unless otherwise noted.

- Wednesday, April 11, 2012 — 4:00 pm (Sullivan clinic)

- Tuesday, May 15, 2012– 11:00 am
Participants must register for orientations as there is limited seating. There is no cost or obligation to attend an orientation. If you would like to attend our orientations, please RSVP by calling us at
